If you think you may require any vaccines for foreign travel please complete our Travel Health Questionnaire. If you are not able to complete the online version then complete the Document Version (Patient Travel Health Questionnaire) and return to the surgery either by email, post or hand. This should be at least 6 weeks before you are due to travel.
Once we have received your completed questionnaire, a telephone consultation will be made for you with the practice nurse. We will text you with the date and approximate time of this consultation.
During this telephone call the nurse will discuss your travel plans, any vaccines you may already have and proposed vaccines required for your trip. The nurse will make an appointment with you to get any vaccines administered.
Some travel vaccines are ordered on a private prescription and incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
