Travel health

If you are travelling abroad you may need to be vaccinated.
The practice can offer Hepatitis A, Typhoid and Tetanus as part of the NHS travel vaccine scheme.

To find out if you require any travel vaccines, please complete the information form below AT LEAST 8 weeks before departure.

ONE FORM NEEDS TO BE COMPLETED PER PATIENT registered at the practice who is travelling.

Once the form is received at the practice, our qualified Travel Immunisation Nurses will review this and your medical record. They will send a text to advise you of any vaccinations required for the country(s) you are visiting. This will include any vaccines currently not available on the NHS.

If any NHS vaccines are required the Nurse will ask you to make an appointment no later than 2 weeks before you travel.

Please note in busy periods, if you submit your form later than 8 weeks before you depart then we may not be able to schedule you an appointment in time.
